Marriage Contract  

Requirements to be complied by the client:

1.     Certificate of No Marriage (CENOMAR)

2.     Legal capacity to Contract Marriage (if the applicant is a foreigner)

3.     Municipal Form Number 68 (Parental Advice)

4.     Municipal Form Number 92 (Parental Consent)

5.     Certified True/Xerox copy of birth certificate of applicants

6.     Parental Consent of applicant is 18 years old but below 21 years old

7.     Parent’s advice if applicant is 21 years old but below 25 years old

8.     At least of the contracting parties must be a resident of the place where the local civil registry office is located

9.     Posting for ten (10) days

10.   Releasing of the License on the eleventh day after the ten (10) days posting

11.   If one of the contracting party is a widow or a widower, submit Death Certificate.

12.   If the applicant is 18 to 24 years old and if both parents are deceased, submit Death Certificate of Parents of the contracting party. The signature of the Legal Guardian (nearest relative) of the applicant is required.

13.   Submit Affidavit of Singleness if applicant is 21 to 24 years old.                                                                                  

Fees:

1.     Applicants both residents of San Jose                               -Php 100.00

2.     If one of the applicants is a Non-resident of San Jose    -Php 150.00

3.     If one of the applicants is a foreigner                                  -Php 500.00

4.     Marriage License                                                                      -Php 100.00

5.     Solemnization Fee                                                                     -Php 200.00

6.     Marriage Counseling Fee                                                         -Php 50.00

7.     Family Planning Seminar                                                           Php 50.00
